Architectural Modification Form - Each association has different requirements as to how long the Board has to approve the modification. To find out the turn around time for your association, please contact the management office. Attach any necessary pictures or drawings to the form and fax or mail back to the management office. Assessment Auto Payment Form - Do you want to avoid writing checks and paying for postage? MC offers the ACH (Automatic Clearing House) Auto Payment program, which utilizes electronic funds transfer, to pay your assessment and other charges in a timely, accurate, and convenient manner. Initial setup takes approximately one billing cycle (15-30 days). Please continue to pay your assessments via check until you receive a statement stating your payment will automatically be debited from your account. Once the automatic debiting commences, your account will be debited on the 1st of the month your assessment is due. Please mail or fax the form back to the management office once it is completed. Paid Assessment Letter
/ Condo Questionaire Requests - If you are selling
or refinancing a unit in an association that MC manages, it is necessary to request a Paid Assessment Letter from the management office.
Often times your mortgage company will request a Condo Questionaire or 22.1
Disclosure as well. MC utilizes the services of Condo Certs for Paid
Assessment Letters, Questionaires, and 22.1 Disclosures. In addition, all
association documents are available for purchase on the site. The website
